How Roman numerals work
Seven letters stand for numbers: I = 1, V = 5, X = 10, L = 50, C = 100, D = 500, M = 1,000. Values are added from largest to smallest (MMXXVI = 1,000 + 1,000 + 10 + 10 + 5 + 1 = 2026), except six subtractive pairs, where a smaller letter before a larger one is taken away: IV = 4, IX = 9, XL = 40, XC = 90, CD = 400, CM = 900. So 1994 is MCMXCIV.
Standard and non-standard forms
No letter is used more than three times in a row, and only the six pairs above subtract. That's why 4 is IV, 99 is XCIX (not IC) and 49 is XLIX (not IL). Clocks often show IIII for 4: the converter reads such forms, gives their value, and shows the standard spelling. Without extra symbols, the largest number is 3,999 (MMMCMXCIX).
Dates in Roman numerals
Each part of the date is converted on its own and kept in the order day, month, year: 16 February 2000 becomes XVI.II.MM, and 4 July 1776 becomes IV · VII · MDCCLXXVI. For a tattoo or an engraving, check the order you want (some people prefer month first) and copy the result exactly, since one wrong letter changes the date.

No. Roman numerals have no zero and no negative numbers, so the converter works from 1 to 3,999.
